We talked about the scalpel and the state of affairs with plastic aesthetic surgery in the Emirates with a surgeon from Switzerland, Dr. Michael Boss. About alternative methods of non-surgical rejuvenation, which are much less known, we asked Dr. Maria Hattar, director of the Dubai Aesthetica Clinic of Aesthetic Medicine. It turned out that each of us, dear women, has a choice.

Plastic aesthetic surgery is not a mysterious phenomenon for a modern person, and we see the results of the work of your colleagues every day. What operations do you most often have to do in the UAE, Dr. Michael?

My main practice is in Switzerland, in the Emirates, I work at the invitation of colleagues from the Swiss clinic of aesthetic medicine of Ras Al-Kheim, so I have here not as wide a circle of patients as at home. But, in principle, women around the world have the same requests: facelift, manipulations with the area around the eyes and surgery to change the shape and size of the chest.

When, in your experience, do modern women begin to think about going under the knife?

If we talk about operations that hide the aging process, then this depends both on the woman herself and on the severity of the problem, although it must be admitted that the average age of our patients decreases every year. Still very young ladies want to make adjustments to their appearance. But there are reasonable limits for everything, and a good doctor will always prefer the well-being of the patient to the desire to earn.

Tell me, Michael, how much does an average plastic surgery cost?

So, offhand, it’s hard to say, each case is individual. For example, I can report that a pair of quality implants for breast augmentation costs in the region of US $ 2,500 - 4,000. The operation is paid separately.

I know that in Russia plastic surgeries and peels are advised to do in the winter, when the sun is less active. What to do with it in the Emirates, where it is sunny all year round ...

There is some truth in this, but there is also some fiction. Of course, after some operations and face peeling, exposure to direct sunlight is contraindicated. But this does not mean that a woman will have to sit in four walls: today there are many sunscreens that protect the skin like a shield. There are reasonable rules of conduct: people who go on such operations are aware that they should follow the postoperative regime for some time. It is unlikely that any of them will lie under a knife with a secret desire the next day to go to sunbathe on the beach.

Take, for example, South America, where aesthetic surgery is consistently popular in the same hot climate as in the UAE.

What, in your opinion, is the level of plastic surgery services in the Emirates?

It all depends on the clinic: I saw excellent departments and excellent specialists. But these are, rather, exceptions to the rule. Most often, plastic services are provided in the range of routine surgical services. There are many plastic surgeons in Dubai, but among them there are few doctors with really high qualifications. It’s hard to find a place where prices match quality.

The clinic in Ras al-Khaimah, where I am conducting operations at the moment, has the necessary material and technical base and qualified personnel. All the doctors are from Switzerland, the management company is also Swiss.

Dubai is still far from the high international standards that I'm used to. Therefore, I have matured the project of opening my own clinic here, which would set a higher bar for the provision of services in this area. I already have an agreement on cooperation on this project with one of the Swiss groups with extensive experience in managing aesthetic surgery clinics. For me, this will be a great opportunity to take Dubai and the entire region to a new level.

Including in the doctor-patient relationship? This is not very good here ...

Of course, after all, the overall outcome of the event largely depends on this! Relations should be built from the very beginning. Often customers do not know exactly what they want. There is a desire to change, to become more beautiful, slimmer, more attractive. The doctor is here to help, suggest.

It is very important to work directly with the patient, communicate with him personally, and not with his papers and x-rays. It is important to inform the patient about the pros and cons of plastic surgery so that he can make an informed decision.

Preliminary communication with the patient also gives both parties a sense of confidence and peace: under such stress as global changes in appearance, it is not always nice to see a stranger in a surgical mask in front of you on the day of the operation!

When the patient and the doctor break up quickly after surgery, this is also wrong. Postoperative maintenance should be carried out by the same team that performed the operation. It is not necessary that the surgeon himself remove the sutures, but specialists familiar to the patient should do this.

All this is an integral part of the work of a good doctor. And this will become part of the philosophy of my Dubai clinic.

By the material and technical base do you mean, for example, the wards for recovery after operations, which are practically absent in the local clinics?

Including: it really surprised me. It does not matter where the patient lives, in the Emirates or not, it is important that he remains in the clinic for some time after the operation. Some people just need privacy so as not to be seen by relatives and friends with patches on their faces.

All these small, at first glance, details form the big picture. So far, Dubai is especially proud of nothing, so I am very enthusiastic about my project. We have been waiting here for a very long time.

I'm not afraid of injections! If necessary - I will prick!

Dr. Maria, like any woman frightened by society, I am concerned about the problems of aging, which, in the literal sense of the word, are obvious ... How is this process going?

The natural aging process is determined by nature itself. With age, degenerative changes in the skin occur - it fades, loses color and elasticity, the structure of collagen fibers, water-salt metabolism are disturbed; the epidermis becomes thinner, and the connective matrix, fastening the skin cells, changes its structure. The processes of natural skin renewal slow down with age: if twenty-year-old girls have a 21-day renewal cycle, then after 30 they renew their skin much less frequently. But wrinkles are not as bad as losing the contour and shape of the face. Therefore, for example, driving a person by the car at the transition, we can quickly determine his age category without even looking: we do not see wrinkles, we only see the contour of the face.

The purity of lines is characteristic of a young face: the face itself, cheekbones, chin. When we age, the face begins to "drain" down, obeying gravity. Many factors contribute to this. A deficiency of essential fatty acids occurs, the strength and elasticity of the muscles of the face decrease, and the bone begins to thin. When the necessary fat begins to leave, and this process starts after 30, the skin becomes too large for the face, begins to sag. Thinning bones make the face flatter, it loses its shape.

Therefore, the young face, which previously had the shape of an inverted triangle, after 40 begins to take the form of a trapezoid.

It’s not a very bright prospect, especially when you consider that it is impossible to resist this ...

To resist - no, but to make adjustments to the work of nature is quite within the power of modern aesthetic medicine. In our clinic, we do not deal with the classic facelift. We are working on the problem of aging comprehensively, taking into account all the factors listed by me above. The methods are also different: for someone it is enough to resort to non-surgical rejuvenation, someone needs to increase the volume of tissues to fill the form surgically.

Let’s do it without a knife! What methods of non-surgical rejuvenation do you consider effective?

There are many methods and all of them are effective in certain conditions, often when combined, the best result is obtained. For example, we offer today both traditional methods - chemical peels, mesotherapy, Botox injections, and more effective ones - cell therapy and radiolifting.

Please tell us more about each of them, indicating the pros and cons ...

Chemical peeling is the process of applying an acid solution to the skin, which removes the stratum corneum, destroys unhealthy skin tissue and stimulates the restructuring of the dermis tissue.

Peelings are good for combating skin aging, especially for removing the effects of photoaging caused by exposure to the hot southern sun and dry climate, which is important for the Emirates. The use of chemical peeling evens the surface of the face, restores its color, removes pigmentation and scars. Peeling acid penetrates deep into the skin, stimulates the synthesis of collagen and elastin. As a result, wrinkles are smoothed out, pores are narrowed. We also offer non-chemical, herbal peels that are equally effective.

Every woman needs to do peeling in a professional clinic. Even microdembrasion does not give such a tangible result, not to mention the use of the belongings at home. All this is ineffective, because it simply removes the topmost layer without affecting the connective matrix, which holds the dead cells in place. Professional peeling is aimed precisely at the destruction of the matrix itself, then unnecessary skin cells, figuratively speaking, themselves "fall off" from the face.

What then does the term "mesotherapy" mean?

Mesotherapy consists in the intradermal administration of a cocktail of drugs, including vitamins of group B, C, nicotinic acid, minerals, amino acids. For each specific case, the individual composition of the "cocktail" is selected. This is a completely painless and effective procedure that encourages restructuring of the dermis, creating greenhouse conditions for cellular metabolism and regeneration, restoring the energy potential of tissues, and improving peripheral microcirculation. Mesotherapy has been used in cosmetology since the 1950s to this day, which proves its effectiveness. Mesotherapy is good for treating acne, hair loss, and other problems.

For best results, you must undergo a series of injections for two months, then make one injection per month to maintain the effect.

What is the good method of rejuvenation with your own cells?

Signs of aging are manifested when the number of cells forming collagen in the second layer of the skin, in the dermis, gradually begins to decrease. It is collagen that creates the network of fibers that form its structure and support. Fibroblasts are specialized cells in the skin responsible for collagen formation.

Cell therapy is based on using the patient's own fibroblasts (healthy young cells) in the right amount to correct skin defects, including wrinkles and scars. The advantage of this therapy is that the result can occur indefinitely, as the cells continue to stimulate collagen production, causing the skin to gradually regenerate.

Since the patient’s "native" cells are used for implantation, the possibility of allergic reactions or rejection is excluded. This is a very effective procedure. For clarity, I ran it on one of my hands to show the difference. (Indeed, the skin on Dr. Maria’s left hand looks much younger than on her right!).

The list of services in your clinic includes photopneumatic therapy and radiolifting. What are these outlandish techniques?

The photopneumatic therapy that we carry out with the Isolaz machine is a significantly modified and improved method of phototherapy. During the procedure, the light pulse should process the point of application of forces. In photoepilation, these are hair follicles, in photo-rejuvenation, skin cell melanin, and in the treatment of vascular networks, blood hemoglobin. Its novelty lies in the application of vacuum along with a flash of broadband light, which gives excellent results due to changes in the optical characteristics of the skin.

Another method - radiolifting - is based on the effect of radio waves on the skin, which starts the processes of collagen production, as a result of which the skin texture improves, its elasticity and saturation increase. The procedure is comfortable and painless, accompanied only by a pleasant sensation of warmth. After the session, the patient can immediately return to her usual lifestyle.

It turns out that youth can be maintained without resorting to the services of a plastic surgeon?

And yes and no ... It all depends on each case. But it is always better to start with non-surgical methods of rejuvenation and only then, when they do not give the desired result, go for surgery.
